What's going on with clients leaving treatment against professional advice?
Notwithstanding the literature on this topic, I am looking for some personal perspectives on the issue of clients leaving treatment early.

 Recently where I work there seems to be a trend with clients leaving AMA. We have noticed that those who are not ready to work a program of recovery will find any excuse to leave. Often what we have experienced is the slightest thing like having to actually do chores in their sober living or a change in their daily schedule causes a client to have an outburst and want to leave treatment. 

We have also noticed that too much downtime in our facility causes complaining and threats and other times we have had AMAs due to too many education groups. 

Try eliciting change talk.  I use the same MI skills for my High Intensity Treatment group (people who were unsuccessful in tradition Level I) as I do for my Readiness for Change group (first time in treatment people), and the results are, high attendance and completion rates.  It's all about leadership.
